1. Different colours match different algorithms (laryngectomy or patent upper airway).
2. Diagram shows the common types of procedure – these can be amended depending on what you do at your hospital commonly.
3. When was the trachy performed? Important information if removing or re-inserting a tube as the stoma will be more likely to close quickly with a recent tracheostomy.
4. Patient identifiers – is this the right patient? Names are often removed, but this depends on your local policy.
5. Any notes on the upper airway. If it took three anaesthetists two hours to get a tube down you need to know!
