The principle of oxygenating the patient always takes priority. However, clinical judgement may dictate that attempts at manipulating or inspecting the tube are justified, especially if the tracheostomy stoma or upper airways are going to be difficult to manage. A tracheal stent can be easily displaced during removal or re-insertion of an airway device.

In the vast majority of cases, if the tube is blocked or displaced and the patient is deteriorating, then the safest action is to remove the tube.
